Download and complete the 2025 MSE Application Workbook before starting the application. 

 Please follow the instructions below to download the workbook

  • Right click on the link to the MSE Application Workbook.
  • Select “Save link as…”
  • Choose a location on your computer to save the file.
  • Click save.
  • Complete application workbook.
  • Upload the saved form at the end of the application form.

3. Board Minutes with motion approving application
